This form enables students who will not be on campus for any amount of time for any reason (students who live far away, for example) during Winter or Spring 2021, to notify the College that they will not need to be part of the arrival and surveillance testing programs.

As with all of our efforts to curb the spread of the COVID-19 virus, we rely on the good judgment and integrity of everyone in the community.  If you misrepresent your status on this form, you may face formal action through the student conduct process.
